A: HOW TO DO ON-LINE APPLICATION?
1.       You have to apply on-line for the OCI application, means the on-line registration form is available on the website that you have to fill it up first and then send the hard copy to the Consulate. Unless you send hard copy of the application to the Consulate with the requisite documents and fees, the Consulate will not come to know about your application and your application will not get processed at all.
2.       Please note that downloaded forms are not accepted repeat not accepted as the data filled by you in the on-line application goes in the world-wide on-line internet OCI system.
3.       After reading all the instructions carefully, you can click on the on-line OCI Registration form. The on-line OCI Registration format is been given below, such form will appear when you click the OCI – Registration link. Sometimes the Internet browser will say that website does not have security certificate, do not worry, accept the same and go ahead.

  • You can apply as an individual or as a family.
  • Family comprises of Husband, wife and two minor  (below 18 years)  children
  • First select one of the options from Individual or Family.

4. When you select “Individual” option the fields in the on-line registration form will appear like first two pages shown below:

When you select the on-line registration form as “Family” option, the on-line registration form will appear like next 4 formats shown below, which will ask data for family of four persons (that is First Applicant, Spouse, Minor Child 1  and Minor Child 2).


Family Group option should be opted if only one of the parents is applying along with one/two minor children or husband & wife are applying otherwise you have to always opt an individual option. If there are more members in the family or children are not minor then their applications should be filed as individuals.

5.       Then select a place of submission or select office: For Chicago, you have to select USA-Chicago; if you select another option, our consulate will not able to process your application as this will be registered under different office registration number.


6.       Please fill all the fields in the on-line registration OCI form, please check once again before pressing SAVE command. Once you press the save command “PART A” of the OCI registration form will appear on the screen. Then give print command in a dialogue box that appears on the screen. After this you will get “Part-A” & “Part-B” printouts. Please note that you have to fill up all the essential blank fields in Part A and part B manually, otherwise your application will be incomplete. Please fill up all the information in BOLD (capital) letters.

Please note that the above forms are sample forms and the names and data given is an imaginary data, do not copy that data. It is essential to have online registration number starting with USACI followed by seven digits like USACI0123407 on the part A of the application form, when you submit the application to the Consulate.


8.       After taking printout of the application, if you find that you had made a mistake while filling the on-line application and there is a wrong data entry, please re-do the on-line registration. Re-doing on-line registration will not create any problems with your OCI application as the Consulate will consider your application only when you send the duly filled hard copy of your OCI application along with requisite documents and fees. When you are re-doing on-line registration, please trash the “Part-A & Part-B” which was generated earlier with wrong data, otherwise that will create confusion.


9.       Please note that the data which enter in the system directly goes in the on-line OCI system and Consulate officials do not interfere with the data. Hence if there are errors in the information being printed in the OCI documents, it is because of applicant’s wrong entries. The Consulate will not be responsible for the same. Thus request for issue of fresh document/visa sticker for name corrections will be entertained only against resubmission of application on line and sending a hard copy, complete in all respects, under OCI Miscellaneous Services from. Do not manipulate the data on the hard copy by any means and then make argument stating that hard copy data shows the correct data and how the Consulate has made a mistake while printing the data.


10.     The main applicant(s) may then fill Part B in capital letters only. Irrespective of number of applicants in Part A, Part B  will be a single form which needs to be signed by the main applicant i.e. if Part-A contains particulars of an individual, he/she should sign at the end of Part B.If Part-A contains particulars of spouses, both should sign at the end of Part B.


11.     Minor children need not sign at the end of Part-B. In the box provided for signature near photo, only applicant should sign or put his/her thumb impression precisely in the box, not spilling out of the box.Parents should not sign in case of minors. If minors can sign, then let them sign. In case of Minors both parents should sign and provide photocopy of their passports (with signature page).


12.     The signature should be made using thick pen with blue or black ink and thumb impression (left thumb for male and right thumb for female) should also be clearly visible.


13.     Do not forget to indicate the date and mode of acquisition of foreign citizenship under column 19 in Part-B.
Please furnish US/foreign passport detail against column 8 and 9 of part A as per your present nationality. When you are applying for OCI card, that means you are not Indian national, hence put your current (US/Foreign) nationality in the on-line registration form.

 

B: HOW TO ARRANGE THE APPLICATION(S)?

 (a) “Individual” Application Option

1. Part-A.
2.Part-B – only one (pl do not include the pages containing instructions after Part B)
3. Proof of present nationality (copy of first two and last two pages of US/foreign passport)
4.Copy of birth certificate – applicable only if applicant is born abroad
5. Copy of the PIO card (if you are holding)
6.Proof of address (copy of utility bill like electricity or telephone bill or driving license where your address is mentioned)
7.Proof of previous nationality (copy of first three and last three pages of last Indian passport held) or any other documentary evidence like a notarized copy of educational certificate issued by an institution in India and land ownership certificate by which eligibility may be reasonably ascertained or a duly notarized affidavit to that effect. The decision of the Consular Officer (deciding your Indian identity) will be final in such cases.

                                                                        OR

If parents’/grand parents’ origin is being claimed as basis for grant of OCI, enclose the following documents:

1. Evidence of relationship with the applicant as parent/grand parent: If you are claiming OCI card on the basis of parent Indian origin then your birth certificate (where parents name is mentioned) is required, if you are claiming the same with your grand-parents’ origin then you have to submit your birth certificate as well as your parents’ birth certificate showing the parents names clearly so that it establishes the relation with your parents and grand parents.

2. Copy of their Indian passport, or

3. Copy of their domicile certificate issue by the competent authority; or any other documentary evidence like a notarized copy of school certificate, land ownership certificate by which eligibility may be reasonably ascertained. The decision of the Consular Officer will be final in such cases.

 

(b) “Family” Application option

1. Part A (First Applicant) + Part A (Spouse)+ Part A (Minor Child 1) + Part A (Minor Child 2)
2. Part B – only one (as only one part B comes with four part A from the system), irrespective of number of applicants under Family Group (pl do not include the pages containing instructions after Part B)
3. Proof of present nationality (copy of first two and last two pages of US/foreign passport)
4. Copy of birth certificate – applicable only if applicant is born abroad.
5. Copy of the PIO card (if you are holding)
6. Proof of address (copy of utility bill like electricity or telephone bill or driving license where your address mentioned)
7. Please note that you need to submit application as follows:

    • One SET of application comprised of items mentioned in 1 to 7 of (a) or (b) above as per the receptive case (Individual or family) and applicable documents.
    • Application needs to be submitted in two such sets.
    • One photo each (as per the specifications given below) should be affixed with glue on part A of two sets. Additional three photos are required; these could be stapled on part A of first set. Thus total five photos are required for one application.
  • In case of minor applying with their parents under “Family Group”, copies of parents’ Indian passports need not be again enclosed with the documents of these children. 
  • In case you are sending application for your minor child as individual, then photocopies of parents OCI cards or US passports & earlier Indian Passports are essential.
  • Fees of all the applicants to be put with Part A of the first applicant only and not with the Part A of respective applicant.
  • Please note that you have to self-attest all the photo copies stating “This is a true copy of the original” and your signature.
  • Make sure that all the photo-copies should be legible (readable), and it is better to send the colour photocopies.  If we are not able to see the contents and recognize the photo on the photocopies, application will be sent back to you without processing.
  • Remember that at the application stage, do not send your original passports or documents, these should be sent only at the time of collection of OCI documents.

The minor children (having US or foreign passports) whose both parents are Indian passport holder are not eligible for OCI card, but they are eligible for PIO card.

RENUNCIATION FORM AND SURRENDER CERTIFICATE:

You must enclose the copy of the Surrender Certificate in both sets of the application for individual OCI applicant apart from the items listed above. If you do not have surrender certificate issued by the Indian Government authorities after seeking foreign nationality, then follow the procedure for applying surrender certificate by clicking the tab below. This is essential to process your OCI application.

C: HOW MUCH IS FEES AND HOW TO PAY?
(1) Fee:
a. In case applicant is  holding PIO Card

i)US citizens $25.00 + mailing fee
(ii) Non-US citizens  $45.00 + mailing fee; (Includes Referral fee of $20.00 is for seeking additional information from Indian Mission/Post in the country of applicant’s citizenship).
(iii) For minors holding PIO card, fee is US$ 145/- + mailing fee

b. Other applicants


(i)US citizens $275.00 + mailing fee
(ii) Non-US citizens $295.00 + mailing fee (Includes Referral fee of $20.00 is for seeking additional information from Indian Mission/Post in the country of applicant’s citizenship).

In both (a) & (b) above

Surrender certificate fee:Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-

 

(2)     Mailing Fee:

i ) Individual applicant US $ 20.00
( ii ) Family : US $ 20.00 for first applicant plus US$ 1.00 for each additional applicant i.e. for family of two persons US$ 21 ($ 20+1); for a family three persons US $ 22.00 ( 20+$1+$1 ); for a family of 4 persons : $ 23.00 ( $ 20+$1+$1+$1 ); However family comprises of Husband, wife & two minor (below 18 years) children only.

If there are 4 applicants in the family, but children are not minor, then mailing fees will be US$ 61.00 (20+1+20+20) as the children applications will be treated as  individual applications.


NOTE:


You can submit your OCI application as well as collect your OCI documents in person between 1130 to 1230 hours on working days. If you want to receive your OCI documents in person, then please attach a letter with the application stating that “I/we will be collecting my/our OCI documents in person from the Consulate when they will be ready; hence I/we is/are not including mailing fees with my OCI application”. Please also mention your on-line registration numbers in such letter. It is advisable to collect the OCI documents in person to discard the possibility of loosing your Passport & OCI documents in the mailing process and it may be mentioned that you get the OCI documents on the same day in an hour or two. If you are seeking your OCI documents by mail then applications received without mailing fee will not be processed and the same will be sent back at applicant(s) risk without any responsibility of loss in mail.In case you are applying as family, you should pay for all four applicants for example "Fees= 275 X 4 = US$ 1100" and "mailing fees=US$ 23 or as per fees conveyed in point (1) & (2) above" Please note that though you are applying as family, you have to pay for individual applicants in the family.

Please do not forget to provide either the surrender certificate or following the procedure for getting Surrender Certificate along with requisite fees for each individual OCI applicant.
You must pay the fees towards OCI, mailing and Surrender certificate separately. However you may pay a combined cashier cheque towards OCI fees for a family. Similarly you may pay combined cashier cheque towards mailing for a family and combined cashier cheque towards Surrender certificate fees. e.g. family of four persons having applications of parents and to minor kids which got naturalized just recently will pay cashier cheque/money order/postal order as follows:

  1. OCI fees: US$ 1100/- (US$ 275 x 4)
  2. Surrender certificate fees: US$ 700 (US$ 175 X 4)
  3. Mailing fees: US$ 23

      
(3)  Mode of Payment:
    
Cashier Check or Money Order or Postal Order drawn in favour of “Consulate General of India, Chicago”. Cashier check/MO/PO should be properly signed and name of the first applicant should be written on it along with on-line registration number.

  • Personal checks / cash / credit or debit cards are not acceptable.
  • You must pay the fees towards OCI, mailing and Surrender certificate separately as mentioned in the note under point (2) above.
  • Do not club together the fee of applicant(s) not covered under Family Group

In the event of dishonoring of Cashier Cheque/Postal Order/ Money order submitted by the Applicant owing to "Stop payment" or by any other reasons, the applicant has to pay the due fees along with the penalties levied by the Bank. Failing to do so by the applicant will attract legal action including cancellation of the service (if provided already). No arguments on this will be accepted..              

Please note that applications not arranged in the above manner, order and instructions and inadequate fees will be returned back un-processed and these will not get priority in processing after you sent it with corrections as they will again go in the queue

  • Consulate will not be responsible for any kind of loss of documents during transit mail. Make sure that while sending your application by mail send by FEDEX or USPS express mail only so that you have the tracking number and you can trace down your application yourself and thus there are less chances of loosing it by the postal agencies

     
D: PHOTOGRAPH SPECIFICATIONS
Please note that while you do on-line application, there is no space for affixing photo or signing, but after completion of the on-line application and taking printout, you will find the appropriate boxes for signature and photos.
A total of five photographs, 3.5 cm X 3.5 cm each, are required for each application.  One each to be pasted (not stapled) on Part-A of application forms (original and duplicate) and remaining three to be attached/stapled as additional photographs on part-A of the first set. 
Please ensure to attach these additional photographs with the first copy of respective application(s) and not in a bunch with the application of first applicant when applying as a group of family.  The specimen and specifications are given below:

  • Photograph should measure 3.5 cm X 3.5 cm without border
  • Full frontal view with all facial features clearly visible
  • Face should be in the center of the photograph and it should occupy 75% of the photo area
  • Top of the head should not be too close to the upper border or it should cut by the border
  • Face should neither be too bright nor too dark. There should be a proper light on the face
  • If face is yellowish or reddish, then photo will be get rejected by the on-line printer at Delhi
  • If wearing spectacles – reflection of light from the glasses should not be visible. If your frames are too big, then make sure that your eyes should be clearly visible in the photo.
  • Facial features should be clearly visible.
  • No visible shadow on the face or background.
  • The background of the photo should not be white.
  • The Background should be of light colour for example sky blue or yellow
  • Do not wear white/ light colored cloths as it blends with light background.
  • Light reflection from hair should not be visible.

 

Home printed digital photographs or photographs with dark background, undersize/oversize photographs and photographs showing sideview are not acceptable. Applications containing such photographs will be sent back at applicant’s cost. all photographs should be similar and with front view and as per the specifications above.

Please do not take the photos at departmental stores and go to proper photo studio so that they can give photo as per the above specifications.

There is possibility that if you provide photo not as per the specifications mentioned above, it will be rejected by the on-line printing facility at Delhi. Please note that no person is rejecting the same, it is the machine which is rejecting. Do not argue on this issue instead provide a good quality photos.

 

Please click below to see the a good sample photo and sample photos which are accepted and rejected by the on-line printing machine so that you can take your photos accordingly. There is one photo with red background, which is accepted in the samples but make sure that you take photo with light background as mentioned above only. Do not show contradictions here.


Please also note that we have mentioned photo size 35 mm x 35 mm in the website. However when you do the on-line registration and print the form, the space for photo will show 51 mm x 51 mm. Please go by the instructions in the website. Do not show this as a contradiction in the information. If you go by the instruction in the website then there will be less chances of not printing of your OCI documents by the on-line printing facility in Delhi.

 

 


 

E: WHERE TO SEND APPLICATION AND THINGS NOT TO FORGET


You have to send/submit a duly filled application to the following address:
“OCI cell, Consulate General of India, Chicago, 455 N. Cityfront Plaza Drive, NBC Tower Building, Suite # 850, Chicago, Illinois-60611”.

Before sending application, please check once again that you have enclosed all necessary documents and appropriate fees & mailing fees etc. write on the top of the envelope “OCI application”. The submission timing at counter is only 1130 to 1230 hours on working days (Chicago time).

Please keep photocopy of your application with you. The on-line registration number on the Part-A of the application is most important for monitoring status of your application. This number will be USACI followed by seven digits e.g. USACI0123675.

No individual acknowledgements are mailed out. The Consulate sends an email after granting of your application. Granting does not mean that your OCI documents are ready with the Consulate. Do not rush and send your US passport to the Consulate before checking on-line that your OCI documents have reached to Chicago.
