A: Why to go for OCI Misc. Services?
This service can be availed for re-issuance/issuance of duplicate OCI documents in the following category of cases:
- After seeking new US/foreign passport after issue of an OCI card
- Change of personal particulars viz. nationality, name, father’s name, date of birth etc.
- Loss / damage of OCI registration certificate (OCI card)/U-visa or loss of US passport (in which OCI visa sticker was affixed)
- In case of change of address/occupation
- After seeking new US/foreign passport
OCI Card and ‘U’ visa sticker contain your US/Foreign passport number. Accordingly, each time when a new passport is issued to an OCI-card holder; it has to be captured on the registration certificate (OCI card) and U visa sticker. This would require re-issuance of OCI Card and U-visa sticker each time a new passport is acquired by the applicant in the following cases:
- Up to the completion of 20 years of age, OCI documents have to be re-issued each time a new passport is issued to the holder as the physical appearance changes.
- Between 21-50 years of age, there is no need to seek re-issuance of OCI documents each time a new passport is issued.
- You can travel with new passport, Old passport (where OCI visa sticker is affixed) and OCI card. It is always better to be on the safer side and get the new OCI card as we often forget to carry our old passport while travelling. This is suggested as OCI card holder will have mental peace.
- After the completion of 50 years of age, OCI documents have to be re-issued once after issuance of a new passport and whenever physical appearance changes.
- Change of personal particulars viz. nationality, name, father’s name, date of birth etc. have changed
In case of change of personal particulars (name, father’s name or date of birth) while submitting online application, OCI card holders may seek re-issuance of OCI documents by availing this services. They have to enclose the documentary evidence for the change in personal particulars requested. In case of change of photograph, personal interview with the Competent Authority will be required before the grant of the service.
- In case of loss/damage of OCI registration certificate/U visa or loss of US passport (in which OCI visa sticker was affixed
In case of loss of OCI documents, a copy of the complaint filed with the Police authorities (where the document is lost) shall be enclosed. Application must be submitted in person and OCI documents should be collected in person.
- In case of change of address/occupation
In case of change of address/occupation, new set of OCI documents will not be issued. The applicant may take a print out of the new address/occupation after online submission and keep for record purposes. No fee shall be charged from the applicant.
Before filling the online application, Guidelines and Instructions for filling application form explained below may be perused so that there is no mistake in submission of the application. Further, the details regarding Fee and Offices where application is to be submitted have also been explained hereafter may also be perused.
The processing time would be around 10 to 14 weeks from the date of receipt of application in the Consulate and it will not be possible to address status queries during this period in this regard.
Applicants are once again advised to read following carefully before accessing application on-line.
1. When you click on the link for OCI miscellaneous service, the screen will appear as follows:

Since you are already OCI-card holder, your data (which is previously filled for OCI application) is already available in the system.
- First you need to fill the current passport number.
- You need to fill in at least one of the following data fields in the application:
- U-Visa number (the number appears at the top right on the OCI visa sticker starting with U)
- OCI Registration number (number which appears on the top of the OCI card starting with A)
- OCI File number (this number appears on the OCI card’s last line of last page
- Thereafter, the applicant has to fill all the following fields:
- Date of birth
- Place of birth
- Mother’s name
Please note that your data should exactly match with the previous data which you filled (when you did last time OCI application). If there is a difference of one bit or space/comma, that will make your present data in difference with the previous data, and then system will not allow you for the next step citing “Information submitted by you does match with the database. Please try again.” The screen will appear like given below:

Do not panic; try to match with the data you filled while applying earlier using the hard copy of that application. If you are not succeeding in matching the data base after several attempts, then call us on 312-595-0415 between 1130 to 1230 hours (Chicago time) on working days or send us an email at (Click Here To Send Email) giving your contact details. This feature has been incorporated as a security measure so that the scheme is not misused and nobody can play with your information.
- Once you are through with this the screen will appear as follows, for continuation press OK.

- After pressing OK, the applicant thereafter has to select any one or more of the following services:
- Change of Passport particulars i.e. No., date/place of issue etc
- Change of Personal particulars i.e. name, date/place of birth etc
- Loss/damage of OCI certificate/U-visa sticker
- Change of Address/Occupation
- After selection of the service(s) to be availed, a Registration Form will appear having only the relevant fields, which need to be entered as per the services (s) sought. The applicant has to fill all the relevant fields afresh online. The reasons for availing the services shall also be filled by the applicant.
- Upon submission of the Registration Form on-line, a hard copy of the application indicating Reference Number USACK followed by 7 digits like USACK0123567 shall be automatically generated by the computer. This will be your on-line registration number. If you have made mistake while filling up the data do the on-line registration once again and then take printout and send the same (corrected version). Do not manipulate the data on hard copy as that will not change the data you saved in the on-line system.
- The applicant has to sign the application form, paste the photograph and append his signature in the box provided and submit two independent sets of the application form along with the relevant enclosures (with both copies of application) and fee as mentioned below. The application can be submitted to the Consulate General of India, Chicago if you have received your OCI card earlier from different Mission.
- As explained in A-4, no hard copy of the application shall be generated in case of change of address/occupation since no application needs to be submitted to the Indian Mission/Post/Office.

C. Documents to be submitted with application
(a) After seeking new US/foreign passport
(i)Two copies of printouts of the online application. The applicant must affix one colour photo [35 mm x 35 mm] on each form and sign in the signature box. For photo specifications please click below
(ii) Two Copies of existing OCI card and U-visa sticker pasted on expired/cancelled US/foreign passport.
(iii)Two copies of new US/foreign passport
(iv)Two copies of address proof in the form of utility bill (electricity / telephone bill) or lease deed, driver’s license copy
(v)Fees in the form of Cashier cheque/ Money Order/Postal order.
(b) In case of change of personal particulars (or due to wrong filling of particulars by the applicant in previous online application)
(i) Point number (i) to (iv) of the above column C (a)
(ii) Two Copies of the document (s) in support of the change requested {for example new Citizenship certificate in case of change of nationality}
(iii) Fees in the form of Cashier cheque/ Money Order/Postal order
(C) In case of loss or damage of OCI card and/or OCI Visa:
- Point number (i) to (iv) of the above C (a)
- Two copies of the detailed police report of the complaint lodged in case of loss of the document or original OCI document with its two copies in case of damaged OCI document
- A note giving details of the circumstances of loss.
- Fees in the form of Cashier cheque/ Money Order/Postal order
(d) In respect of change of address/occupation –
No hard copy of the application shall be submitted to the Consulate. Accordingly, the issue of Payment of fee does not arise.
Please note that in case you are sending application for your minor child as individual, then photocopies of parents OCI cards or US passports & earlier Indian Passports are essential.
D. How much is Fee & how to make Payment ?
(i) OCI document fees
(a) Issuance of OCI documents when you sought New US/foreign passport or there is a change of personal particulars: US$25
(b) Issuance of duplicate OCI documents in case of loss/damage of old OCI card or US/Foreign passport carrying OCI Visa sticker:
US$ 100
(c)Surrender certificate fee:Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-.
(ii) Mailing Fee:
i ) Individual applicant US $ 20.00
( ii ) Family : US $ 20.00 for first applicant plus US$ 1.00 for each additional applicant i.e. for family of two persons US$ 21 ($ 20+1); for a family three persons US $ 22.00 ( 20+$1+$1 ); for a family of 4 persons : $ 23.00 ( $ 20+$1+$1+$1 ); However family comprises of Husband, wife & two minor (below 18 years) children only.
If there are 4 applicants in the family, but children are not minor, then mailing fees will be US$ 61.00 (20+1+20+20) as the children applications will be treated as individual applications.
NOTE:
You can submit your OCI application as well as collect your OCI documents in person between 1130 to 1230 hours on working days. If you want to receive your OCI documents in person, then please attach a letter with the application stating that “I/we will be collecting my/our OCI documents in person from the Consulate when they will be ready; hence I/we is/are not including mailing fees with my OCI application”. Please also mention your on-line registration numbers in such letter. It is advisable to collect the OCI documents in person to discard the possibility of loosing your Passport & OCI documents in the mailing process and it may be mentioned that you get the OCI documents on the same day in an hour or two. We follow Chicago time.
Please also note that if you are sending OCI Misc application only for minor child then that should also include two copies of the 'OCI cards of the parents' or 'US & Indian Passport copies of parents' apart from the above-mentioned documents.
If you are seeking your OCI documents by mail then applications received without mailing fee will not be processed and the same will be sent back at applicant(s) risk without any responsibility of loss in mail.
- Mode of Payment:
Cashier Check or Money Order or Postal Order drawn in favour of “Consulate General of India, Chicago”. Cashier check/MO/PO should be properly signed and name of the first applicant should be written on it along with on-line registration number.
- Personal checks / cash / credit or debit cards are not acceptable.
- Application fee plus mailing fee can be paid together through one CC/MO/PO by an individual applicant or family group.
- Do not club together the fee of applicant(s) not covered under Family Group
Dishonored Cashier Cheque / MO / PO attracts penalty. If this happens, applicant has to pay due fees as well as the penalty levied by the bank, otherwise applicant will face legal action including cancellation of the OCI card (if issued already).
Please note that application not arranged in the above manner, order and instructions and inadequate fees will be returned back un-processed. This will not get priority in processing after you sent it with corrections as this will again go in the queue.
- Consulate will not be responsible for any kind of loss of documents during transit mail. Make sure that while sending your application by mail send by FEDEX or USPS express mail only so that you have the tracking number and you can trace down your application yourself and thus there are less chances of loosing it by the postal agencies.
E. Where to send/submit application?
You have to send a duly filled application to the following address: “OCI cell, Consulate General of India, Chicago, 455 N. Cityfront Plaza Drive, NBC Tower Building, Suite # 850, Chicago, Illinois-60611”.
You can submit your application in person also between 1100 to 1230 hours (Chicago time) on working day. Before sending application, please check once again that you have enclosed all necessary documents and appropriate fees & mailing fees etc. write on the top of the envelope “OCI Misc Service application”.
Please do not send your original passports at this stage.
Please keep photocopy of your application with you. The on-line registration number of the application is most important for monitoring status of your application. This number will be USACK followed by seven digits e.g. USACK0123675. No individual acknowledgements are mailed out. The Consulate sends an email after granting (approving) of your application. Granting does not mean that your OCI documents are ready with the Consulate. Do not rush and send your US passport to the Consulate before checking on-line that your OCI documents have reached to Chicago.
