If you want to carry out the following changes by way of endorsement (observation by the Consular Officer) in the passport, then apply for “Endorsements in Passport”.

      1.     Addition or deletion of the spouse name,
      2.     Change in the permanent address in India or USA address,
      3.     Emigration Check Not Required (ECNR)

1.       Application form
Misc. Services form duly filled in and signed (no column left blank) should be submitted. In case of those who can not sign, thumb Impression (Right Hand Thumb Impression for female and LHTI for male). Please click below for downloading Misc. Services form.


2.       Photographs
Two recent colour photographs of 2” x 2” against white background showing full frontal view of face are required. Out of these one photograph is to be pasted (do not staple this photo) on the first page of the application in the space provided for the purpose and other one should be stapled at different place. Photographs with other than white background will not be accepted.

3.       Existing original passport & copies of first three and last two pages of the passport and any other page containing Passport Office’s observation. Please note that for DOB endorsement you have to come personally and Consular officer will interview you; hence applications will not be accepted by mail.

4.       Proof of residence: 
Any two of these i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your address in USA is clearly mentioned should be provided. Photocopy should be duly notarized, if you are submitting application by mail.

5.       Visa Status
Copy of Permanent Resident Card (both sides) or Employment authorization card or copy of page showing H1 / H4, L1/L2, J1/J2 & F1/F2 etc. visa should be enclosed. If you have applied for visa extension, then such proof should be provided.

6.       Fees
For each endorsement: US$ 10/-
For Mailing Fee: US$ 20/- for USPS Express Mail.  

You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago”. When you are depositing application in person at the Consulate, then only you can pay fees in the Cash, otherwise not. You can pay only one (combined) MO / PO / CC for both “Fees towards Passport” & “Mailing Fees”. When you are paying in MO/PO/CC, then pay full amount by the same, otherwise pay full in cash. Partial payments by cash or by MO/PO/CC are not acceptable.
Credit/debit cards/personal Cheque are not accepted.

7. Processing Time:

Submitted at counter 9:00 to 12:30 hours:           5 to 7 working days
Applied by mail:                                                         30-45 days
Submitted at counter 9:00 to 12:30 hours & requested delivery by mail: 21 working days

(a)     Addition or deletion of spouse name: Copy of Marriage Certificate or Divorce Decree or Death Certificate (as the case may be). Photocopy should be duly notarized, if you are submitting application by mail.

(b)      Change in the permanent address in India or in USA: Proof of new address i.e. copy of utility bill or rent agreement or bank document or ID or Driving License etc showing your address in India or in USA (notarized if applying by mail).
 
(c)     Emigration Check Not Required (ECNR): Notarized copy of Degree certificate / proof of stay abroad for more than 3 years or 10th Grade pass / “O” level & above pass or Govt. of India recognized board or its equivalent certificate is required.

Please also note that notarization is not required if you are submitting application in person, but in that case you have to show the original passport as well as other documents to the Consular Official.
Photocopy should be duly notarized, if you are submitting application by mail.
We follow the Chicago time.
Please note that when you are sending application by mail then send it by USPS Express mail or FEDEX only so that you can trace your application with the postal agency. Please note that the Consulate General of India, Chicago will not bear any responsibility for loss of documents during transit in mail.
Incomplete applications are sent back at applicant’s cost without prior notice.
The decision of the Consular Officer will be final in all cases.
