If you want to change your name (due to Marriage or Divorce) then apply for “Reissue of Passport with name change (Marriage/Divorce)”. Following documents are required.

 

1.       On Line application (Please note that downloaded forms are no longer accepted)
Please go through carefully through the instructions given in the Tab “Passport on-line application procedure” and then click for Reissue of Passport on-line application below:

Duly filled and signed by the applicant (no column left blank) should be submitted.  Please indicate name after marriage in column 1(a), maiden/existing name against 1(d), change of name prior to marriage, if any, may be indicated at 1(c). In case of those who can not sign, thumb Impression (Right Hand Thumb Impression for female and LHTI for male) should be provided.

2.       Photographs
Four recent colour photographs of 2” x 2” against white background showing full frontal view of face should be provided. Out of these one photograph is to be pasted (do not staple this photo) on the first page of the application in the space provided for the purpose. Photographs with other than white background will not be accepted.

3.       Existing original passport & copies of first three and last two pages of the passport and any other page containing Passport Office’s observation should be enclosed.

4.       Photocopy of Spouse’s passport: First three and last two pages of the passport and any other page containing Passport Office’s observation should be enclosed.

5.    Photocopy of Marriage Certificate or divorce court decree: If applying by mail, then photocopy of marriage/divorce decree should be duly notarized. 

6.       Affidavit for Change of name:  
Please click tab below for this affidavit. A joint photograph of 2” x 2” is required to be pasted on the affidavit. If applying in person, affidavit need not be notarized but can be signed by both in the presence of the Consular Officer, otherwise it should get notarized.  This affidavit is not required if you are changing the name due to divorce.

7.       Proof of residence: 
Any two of the following i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be enclosed. If applying by mail, then photocopy should be duly notarized. 

 
8.       Visa Status
Copy of Permanent Resident Card (both sides) or Employment authorization card or copy of page showing H1 / H4, L1/L2, J1/J2, F1/F2 etc. visa should be enclosed. If you have applied for visa extension, then such proof should be provided.

 

9.       Fees
For Passport:  Adult: US$ 40/- 

Reference Fees: US$ 20/- should be paid if the existing passport is not issued by the Consulate General of India, Chicago

Affidavit notarization: US$ 10/-

For Mailing Fee: US$ 20/- for USPS Express Mail.  

You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago”. When you are depositing application in person at the Consulate, then only you can pay fees in the Cash, otherwise not. You can pay only one (combined) MO / PO / CC for both “Fees towards Passport” & “Mailing Fees”. When you are paying in MO/PO/CC, then pay full amount by the same, otherwise pay full in cash. Partial payments by cash or by MO/PO/CC are not acceptable.
Credit/debit cards/personal Cheque are not accepted.

10. Processing Time:

Submitted at counter 9:00 to 12:30 hours:           5 to 7 working days
Applied by mail:                                                         30-45 days
Submitted at counter 9:00 to 12:30 hours & requested delivery by mail: 21 working days

NOTE

Please also note that notarization is not required if you are submitting application in person, but in that case you have to show the original passport as well as other documents to the Consular Official. Otherwise all photocopies should get notarized.

Incomplete applications are sent back at applicant’s cost without prior notice.  Applications received by mail will not be entitled for emergency services. 

We follow the Chicago time.

Please note that when you are sending application by mail then send it by USPS Express mail or FEDEX only so that you can trace your application with the postal agency. Please note that the Consulate General of India, Chicago will not bear any responsibility for loss of documents during transit in mail.

You can get your passport on the same day in case of emergency (serious illness or death in the family in India) by paying additional Tatkal fees of US$ 150/-. This emergency should be supported by the requisite documents (like fax/email about hospitalization/death certificate or communication from the family/hospital). Please also note that in case of Tatkal services and in which references are required, passport will be issued for one year validity only.

The decision of the Consular Officer will be final in all cases.
