The following is the requirement of POA attestation.
Please note that for attestation of the POA the applicant has to come in person in the Consulate and has to sign in front of the Consular Officer. Hence POA attestation application by mail will not (repeat NOT) be accepted/entertained by the Consulate. POA document(s) is/are attested by the Consulate only if these are to be produced in INDIA only.
1. Application form
Misc. Services form, duly filled in and signed (no column left blank) should be submitted. In case of those who can not sign, thumb Impression (Right Hand Thumb Impression for female and LHTI for male). Please click below for downloading Misc. Services form.
2. Photographs
Two recent colour photographs of 2” x 2” against white background showing full frontal view of full face are required. Out of these one photograph is to be pasted (do not staple this photo) on the first page of the application in the space provided for the purpose and other one should be stapled at different place. Photographs with other than white background will not be accepted.
3. Existing original passport & copies of first three and last two pages of the passport and any other page containing Passport Office’s observation. If executants are US citizen then copy of first three and last two pages should be provided and these copies should be duly notarized.
4. Proof of residence:
Any two of these i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be provided.
5. Visa Status:
Copy of Permanent Resident Card (both sides) or Employment authorization card or copy of page showing H1 / H4, L1/L2, F1/F2 & J1/J2 etc. visa should be enclosed. This is not required for US citizens.
6. Power of Attorney Documents:
Two copies with a photograph of the Executants duly affixed on each copy, near the place of his/her signature at the end of the document. Please note that you have to sign in front of the Consular Officer in the Consulate.
All commercial documents should be apostilled by the Secretary of the State of the respective state (in the USA) before submission to the Consulate. Hence there is no requirement of signing in front of the Consular Officer.
7. Fees:
Please note that the fees given below is for each photo/signature attestation
- Property related: US$ 20/-
- Non-property related: US$ 10/-
- Commercial: US $ 50/-
- Surrender certificate fee: Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-
You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago”, or in cash.
Credit/debit cards/personal Cheque are not accepted. When you are paying in MO/PO/CC, then pay full amount by the same, otherwise pay full in cash. Partial payments by cash or by MO/PO/CC are not acceptable.
8.PROCESSING TIME:
Submitted at counter between 900am to 1230 pm: Delivery Same Day between 330 pm to 500 pm

Role of the Consulate is restricted to the attestation of applicant’s signature with no responsibility of contents. We follow the Chicago time. The decision of the Consular Officer will be final in all cases.
