The following is the requirement for issuance of the Police Clearance Certificate (PCC) when passport is issued by the Chicago Consulate less than 5 years ago or the passport issued by the other Passport issuing authorities. You can submit the PCC application and collect the PCC in person as well as by mail.
Please note that this application procedure is comprised of two stages.
Stage 1
1. Application form
Misc. Services form, duly filled in and signed (no column left blank) should be submitted. In case of those who can not sign, thumb Impression (Right Hand Thumb Impression for female and LHTI for male). Please click below for downloading Misc. Services form.
2. Photographs
One recent colour photograph of 2” x 2” against white background showing full frontal view of full face is required. This should be pasted (do not staple) on the first page of the application in the space provided for the purpose...
3. Photocopy of Passport: Copies of first three and last two pages of the passport and any other page containing Passport Office’s observation. Please do not send your original passport at the first stage.
4. Proof of residence:
Any two of these i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be provided.
5. Visa Status:
Copy of Permanent Resident Card (both sides) or Employment authorization card or copy of page showing H1 / H4, L1/L2, F1/F2 & J1/J2 etc. visa should be enclosed.
6. Fees:
(i) Fee: US$ 20/- (for reference
)
Surrender certificate fee: Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-.
(ii) You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago”. You can pay in cash, only if you are submitting your application in person at the Consulate. When you are paying in MO/PO/CC, then pay full amount by the same, otherwise pay full in cash. Partial payments by cash or by MO/PO/CC are not acceptable. You can send one or more application by mail, for every additional application add one dollar mailing fee per applicant.
Credit/debit cards/personal Cheque are not accepted.
7. Processing Time: Waiting period of 60 days after receipt of the duly filled application in the Consulate.
Stage 2
1. Application form:

3. Original Passport: This is required at stage 2 for making observation in the Passport about the issuance of the PCC.
4. Fees:
Fee: US$ 20/-
Mailing Fee: US$ 20/-
Surrender certificate fee: US$ 175/- (if you are not enclosing the copy of the surrender certificate or
renunciation certificate attested by the Indian Government Authorities, then only). This is applicable only in case of naturalized citizens of Indian origin and only one time. All the instructions mentioned in Stage 1 Point 6 (ii) are applicable here also. You can give a combined Cheque towards mailing fees and PCC fees.
5. PROCESSING TIME:
Submitted at counter between 900am to 1230 pm: Delivery Same Day between 330 pm to 500 pm (Chicago time)

NOTE:
Please also note that notarization is not required if you are submitting application in person, but in that case you have to show the original passport as well as other original documents to the Consular Official.
If you are submitting application by mail then send duly notarized copies.
Please note that when you are sending application by mail then send it by USPS Express mail or FEDEX only so that you can trace your application with the postal agency. While sending application for PCC by mail, write on the top of the envelope “For PCC”.
Please note that the Consulate General of India, Chicago will not bear any responsibility for loss of documents during transit in mail. We follow the Chicago time.
While seeking PCC by mail, please write on the top of the envelope “For PCC”.
The decision of the Consular Officer will be final in all cases.
