If Indian citizen does not posses any travel document and there is an emergency to go to India, “Emergency Certificate” service can be availed. This is one way travel document only for going to India.
Please note that this service is not (repeat NOT) available by mail and you have to come in person for submission of the application, interview with Consular Officer and for collection of ready Emergency certificate.
Please note that EC for one month validity is being issued and you have to travel to India before its expiry. Following is the requirement for seeking EC.
1. Application form
You have to apply for an EC on-line (Please note that downloaded form will not be accepted). Duly filled and signed by the applicant (no column left blank) should be submitted. In case of those who can not sign, thumb Impression (Right Hand Thumb Impression for female and LHTI for male) should be provided.
Please go through carefully through the instructions given in the Tab “PassportOnlineProcedure” and then click for EC On-line application below. Please note that you have to select “Re-issue of Passport” option in the desired service column:
2. Photographs
Six recent colour photographs of 2” x 2” against white background showing full frontal view of face. Out of these one photograph is to be pasted (do not staple this photo) on the first page of the application in the space provided for the purpose. Photographs with other than white background will not be accepted.
3. Lost Passport copies or original expired passport: First three and last two pages of the passport and any other page containing Passport Office’s observation.
4. Proof of residence:
Any two of the following i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be provided.
5. Fees:
For EC: US$ 5/-
For reference to other authorities: US$ 20/- (if required only)
You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago”. You can pay only one (combined) MO / PO / CC for all. When you are paying in MO/PO/CC, then pay full amount by the same, otherwise pay full in cash. Partial payments by cash or by MO/PO/CC are not acceptable.
Credit/debit cards/personal Cheque are not accepted.
6. Detailed Police Report: This is must in case of lost passport case. You need to lodge complaint with the Police immediately after the lost or theft of your Passport to avoid the misuse of your passport.
7. Affidavit
You have to fill up the affidavit along with the on-line application form for ‘EC on-line application’. Please download the Affidavit form below by clicking the tab:
8. Personal Particulars Form (PPF): Please click below for PPF. You need to submit the same in 4 copies duly filled.
9.Statement: You should submit a statement mentioning the reasons for seeking EC.
10. Indian identity documents:
If passport is not available the documents proving your Indian-ness should be provided e.g. birth certificate, school leaving certificate, election ID card or any other Government identity card/document.
11.PROCESSING TIME: Depending up on the response of the authorities in India.
