If Indian citizen have been issued an EC and due to some un-avoidable circumstances like serious illness etc. he/she was not able to travel to India during the prescribed period mentioned in the EC, then the person can avail this service.
Please note that this service is not (repeat NOT) available by mail and you have to come in person for submission of the application, interview with Consular Officer and for collection of Duplicate Emergency certificate.
Please note that EC will be renewed for one month validity only and you have to travel to India before its expiry. Following is the requirement for seeking renewal of an EC.
1. Application form
You have to apply for Misc. Service form. Duly filled and signed by the applicant (no column left blank) should be submitted. In case of those who can not sign, thumb Impression (Right Hand Thumb Impression for female and LHTI for male) should be provided.

One recent colour photographs of 2” x 2” against white background showing full frontal view of face. This is to be pasted (do not staple) on the first page of the application in the space provided for the purpose.
3. EC: You should provide original EC along with its photocopy.
4. Copy of the air-ticket: This is must for renewal of the EC.
5. Proof of residence:
Any two of the following i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be provided.
6. Fees:
For EC: US$ 5/-
Surrender certificate fee: Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-.
You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago” or in cash.
Credit/debit cards/personal Cheque are not accepted.
7. Statement:
You should submit a statement mentioning the reasons for not traveling during the prescribed period and need to submit the supporting documents for the same.
8. PROCESSING TIME: Submission at Counter between 900 to 1230 hours: Delivery at 1530 to 1700 hours. (if issued earlier by this Consulate)
