The following is the requirement for attestation of documents required for transportation of dead body to India. Please note that this application can be submitted in person as well as by mail. It is desirable to get the original certificates attested by the consulate to ensure clearance through customs and immigration check points in India.

This form should be duly filled in and signed (no column left blank) for submission. In case of those who can not sign, thumb Impression (Right Hand Thumb Impression for female and LHTI for male).
2. Photographs
Please note that if the passport of the deceased is available then no photos are required. Otherwise the Consulate issues an Emergency Certificate and for that 4 colour photographs showing full frontal view of face of the dead person are required.
3. Existing original passport of the deceased & copies of first three and last two pages of the passport and any other page containing Passport Office’s observation. The passport will be cancelled and returned back.
4. Intimation of death – A relative or friend of the deceased should give a duly signed statement intimating the death. The person intimating death should provide a copy of his/her passport (first two and last two pages only), copy of his Driving License or State I.D. or utility bill or any other document showing complete address.
5. Death Certificate: Original Death Certificate (issued by the County or respective department of State) along with the copy should be submitted.
6. Embalming Certificate: Original Embalming Certificate (issued by the Funeral Home) along with the copy should be submitted.
7. No Communicable Disease Certificate: Original “No Communicable Disease Certificate” (issued by the hospital or doctor) along with the copy should be submitted.

Please note that when you are sending application by mail then send it by USPS Express mail or FEDEX only so that you can trace your application with the postal agency. Please note that the Consulate General of India, Chicago will not bear any responsibility for loss of documents during transit in mail.
Credit/debit cards/personal Cheques are not accepted.
9. PROCESSING TIME:
Submitted at counter: Immediately while the person (submitting) is waiting across the Counter
Mail: Mailing time only.

NOTE:
Please also note that notarization is not required as you will be submitting original documents also in both cases i.e. submission in person as well as by mail.
