The following is the requirement for issuance of the NORI certificate. You can submit the NORI application and collect the NORI certificate in person as well as by mail.
Please note that this application procedure is done in two stages.
Stage 1
1. Application form
Misc. Services form duly filled in and signed (no column left blank) should be submitted.

2. Photographs
One recent colour photograph of 2” x 2” against white background showing full frontal view of full face is required. This should be pasted (do not staple) on the first page of the application in the space provided for the purpose.
3. Photocopy of Passport: Copies of first three and last two pages of the passport and any other page containing Passport Office’s observation. Please do not send your original passport at the first stage. If you are submitting application by mail then send duly notarized copies.
4. Proof of residence:
Any two of these i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be provided. If you are sending application by mail then copies should be duly notarized.
5. Visa Status:
Copy of Permanent Resident Card (both sides) or Employment authorization card or copy of page showing H1 / H4, L1/L2, F1/F2 & J1/J2 etc. visa should be enclosed. If you are sending application by mail then copies should be duly notarized.
6. NORI form & Affidavit:
Please submit duly filled four copies of the Affidavit with the application. Please click below to download the Affidavit. You need to submit three copies of the duly filled NORI form. 7. Fees:
- Fee: US$ 60/- (for reference)
- Mailing fees: US$ 20/-
- Surrender certificate fee: Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-.
You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago”. You can pay in cash, only if you are submitting your application in person at the Consulate.
Please note that when you are sending application by mail then send it by USPS Express mail or FEDEX only so that you can trace your application with the postal agency. While sending application for NORI by mail, write on the top of the envelope “For NORI”. Please note that the Consulate General of India, Chicago will not bear any responsibility for loss of documents during transit in mail.
Credit/debit cards/personal Cheque are not accepted. When you are paying in MO/PO/CC, then pay full amount by the same, otherwise pay full in cash. Partial payments by cash or by MO/PO/CC are not acceptable. You can send one or more application by mail, for every additional application add one dollar mailing fee per applicant.
Note
Please note that applicant has to obtain no objection certificate directly from concerned Passport Office, State Government and Ministry of Human Resource Development in India. The Consulate does not follow up with these agencies.
In addition to above three No objection certificates, you have to obtain a third party bar-code number from the US Department of State. This number is a file number for conversion of J1 visa to H1 visa.
After receipt of these no objection certificates from these three agencies, please proceed for Stage 2.
8. Processing Time: Till Applicant submits all the NOCs and bar code number as mentioned above.
Stage 2

4. No Objection Certificates & Bar Code number:
Please submit original No Objection Certificate received from the concerned Passport Office, State Government and Ministry of Human Resource Development in India. Please also provide the bar-code number as mentioned in the stage 1 note.
5. Fees:
Fee: US$ 20/-
Mailing Fee: US$ 20/-
All the instructions mentioned in Stage 1 Point 7 (3) are applicable here also. You can give a combined Cheque towards mailing fees and NORI fees. While seeking NORI by mail, please write on the top of the envelope “For NORI”.
6. PROCESSING TIME:
Submitted at counter between 900am to 1230 pm: Same Day between 330 pm to 500 pm (Chicago Time)

NOTE:
Please also note that notarization is not required if you are submitting application in person, but in that case you have to show the original passport as well as other original documents to the Consular Official.
