The following is the requirement for issuance of the Statement of Need certificate as well as its renewal.
Please note that for getting the Statement of Need certificate first time, you need to submit the application in person only so that Consular Officer will interview you, however you can seek your Statement of Need certificate by mail.
Please also note that while renewing the already issued Statement of Need certificate, you can submit the application and collect the Statement of Need certificate in person as well as by mail.
1. Application form
Please click tab below for Misc. Services form. Duly filled in and signed (no column left blank) should be submitted.
2. Photographs
One recent colour photograph of 2” x 2” against white background showing full frontal view of full face is required. This should be pasted (do not staple) on the first page of the application in the space provided for the purpose.
3. Photocopy of Passport: Copies of first three and last two pages of the passport and any other page containing Passport Office’s observation. If you are submitting application by mail then send duly notarized copies.
4. Proof of residence:
Any two of these i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be provided. If you are sending application by mail then copies should be duly notarized.
5. Visa Status:
Copy of Permanent Resident Card (both sides) or Employment authorization card or copy of page showing H1 / H4, L1/L2, F1/F2 & J1/J2 etc. visa should be enclosed. If you are sending application by mail then copies should be duly notarized.
6. No Objection Certificate:
No Objection Certificate from Ministry of Health (in original), should be provided with the application. This is mandatory for seeking Statement of Need for the first time. Applicant has to get the same from M/o Health.
7. Contract: The Contract with the Hospital/Institute on the standard Agreement Form with a letter from the Program Director on the official letterhead with seal and contact telephone number should be provided. The copy should be notarized.
8. Written Assurance: Written assurance about going back to India after completion of the training should be provided. Please click the tab below to down load the format of the Written Assurance.
9. Bio-data: A copy of Bio-data with address, telephone number, e-mail address/fax number
10. Fees:
- Fee: Gratis (no fees)
- Mailing fees: US$ 20/-
- Surrender certificate fee: Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-.
You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of
“Consulate General of India, Chicago”. You can pay in cash, only if you are submitting your application in person at the Consulate.
Please note that when you are sending application by mail then send it by USPS Express mail or FEDEX only so that you can trace your application with the postal agency. While sending application for ‘Statement of Need’ by mail, write on the top of the envelope “For Statement of Need”. Please also note that the Consulate General of India, Chicago will not bear any responsibility for loss of documents during transit in mail.
Credit/debit cards/personal Cheque are not accepted. You can send one or more application by mail, for every additional application add one dollar mailing fee per applicant.
11. Processing Time:
Counter (900 to 1230 hours) submission: Delivery 1530 to 1700 Hours. (Chicago time)

NOTE:
We follow the Chicago time. Please also note that notarization is not required if you are submitting application in person, but in that case you have to show the original passport as well as other original documents to the Consular Official. The decision of the Consular Officer will be final in all cases.
