ANNOUNCEMENT FOR OCI APPLICANTS OF 2004-05 WHO HAVENOT YET BEEN GRANTED O C I |
During late 2004 and early 2005 a total of 465 persons of Indian origin, holding foreign passports, applied for grant of OCI status, through this Consulate. OCI documents have already been issued to a majority of such applicants. However, a number of applications could not be processed for grant of OCI due to various reasons like incomplete information/documents furnished, columns left blank in the application etc.
2 As a result of vigorous efforts put in by this Consulate for an early decision in the case of applicants who have not so far been granted OCI, it has now been decided by the concerned authorities in India to seek fresh applications from such applicants, in the current format WITHOUT ANY FEE. All concerned are therefore requested to forward the duly filled in and signed application form (two copies) in the current format, alongwith supporting documents (indicated below). For downloading the current format of the application please visit the site: http://www.mha.nic.in/oci/OCIForm.pdf Following supporting documents need to be enclosed with each set of application:
A. Documents to be enclosed with each set of application:
Proof of present citizenship (self attested copy of first two and last two pages of foreign passport containing details of the applicant).
Proof of Indian citizenship – self attested copy of first two and last 2 pages of Indian passport containing details of applicant. OR
if parents/grand parents origin is being claimed as basis for grant of OCI, enclose the following documents:
a.
evidence of relationship with the applicant as parent/grand parent ;b.
copy of their Indian passport, orc.
copy of their domicile certificate issued by the competent authority, ord.
any other documentary evidence like a school certificate (original and a copy), land ownership certificate, birth certificate (original and a copy) etc by which eligibility may be reasonably ascertained. Original documents will be returned after verification by the Consulate.Copy of PIO card, if held by the applicant.
Self Addressed Stamped Envelope (No.10) – to mail out acknowledgement slip.
Three additional photographs (35 mm x 35 mm) of the size specified on the form
.B. PHOTOGRAPH:
The photograph should meet the following specifications:
A total of five photographs, 3.5 cms X 3.5 cms, are required for each application. One each to be pasted (not stappled) on application forms (original and duplicate) and remaining three to be attached as additional photographs. Please ensure to attach these additional photographs with the first copy of respective application(s) and not in a bunch with the application of first applicant when applying as a group of family. The specimen and specifications are given below:

If wearing spectacles – reflection of light from the glasses should not be visible.o
Facial features should be clearly visible.o
No visible shadow on the face or background.o
Do not wear white/ light colored shirt as it blends with light background.Light reflection from hair should not be visible.
NOTE: HOME PRINTED DIGITAL PHOTOGRAPHS OR PHOTOGRAPHS WITH DARK BACKGROUND, UNDERSIZE/OVERSIZE PHOTOGRAPHS & PHOTOGRAPHS SHOWING SIDEVIEW WILL BE REJECTED BY THE SYSTEM.
Hard copies of applications would be accepted, by mail or in person, only from the applicants of 2004-05 lot who have not been granted the OCI status so far. If applying by mail, please ensure to mark in bold letters on the envelope "OCI PENDING APPLICATION 2004-05" and send it to: Consulate General of India, OCI Cell, 455 N Cityfront Plaza Drive, NBC Bldg Tower, Suite # 850, Chicago Illiniois 60611
Before forwarding the application please ensure the following:

Do not sign on behalf of the applicant (including a minor child), in the box near the photograph, in Part A.·
Do place thumb impression (Right Thumb for female and Left Thumb for male) in center of the box, if the applicant (including minor child) cannot sign.·
Do not place both, signature and thumb impression in the box. It should contain either signature or thumb impression of the applicant.·
Don’t staple photograph in Part A. Photo should be pasted. Additional photographs may be stapled.·
Do not use assorted photographs. All photographs should be identical.·
Do not use photographs (i) with side views and (ii) digital photographs printed at home.·
Don’t obtain Thumb impression in black ink. Thumb impression be obtained in blue/navy blue ink.·
Do make two independent sets of application, each containing: (i) application form, (ii) photocopy of foreign passport (first 2 and last 2 pages), (iii) photocopy of Indian passport (first 2 and last 2 pages) , (iv) photocopy of PIO card, if held (v) photocopy of birth certificate (in case of minor born abroad), (vi) photocopy of Naturalisation Certificiate (vii) photocopy of document on the basis of which OCI is claimed, if unable to furnish copy of last Indian Passport, (viii) photo copy of passport of parents/grand parents, if their origin is being claimed for grant of OCI.·
Do enclose a self-addressed-stamped envelope (No.10) – for mailing out acknowledgement slip.·
Do indicate in bold letters "OCI PENDING APPLICATION – 2004-05" on the envelope containing above documents.·
Do watch Consulate’s website each time it is updated for new additions.
